PRIVACY POLICY

Ambulance USA LLC d/b/a Air Medical Transport ("AMT," "we," "us," or "our") arranges medical transportation worldwide. This Privacy Policy explains how we handle personal information through www.AirMedical.com, communications, payments, and transport coordination. Our Terms and Conditions and each accepted Transport Contract govern our services. When AMT is required by the Health Insurance Portability and Accountability Act and related regulations ("HIPAA") to provide a Notice of Privacy Practices, Section 5 applies to Protected Health Information ("PHI") maintained by AMT in that capacity.

1. Information We Collect

Depending on your interaction with AMT, we may collect:

• Identity and contact information, including names, dates of birth, addresses, telephone numbers, email addresses, emergency contacts, identification and travel documents, and relationship to the Patient.
• Medical and transport information, including medical records, diagnoses, medications, mobility needs, required equipment, fit-to-fly information, origin and destination, receiving-facility details, and communications with air, ground, and medical providers.
• Payment and insurance information, including payer details, billing addresses, insurance information, transaction records, and payment authorizations. Payment-card information submitted online is processed by the payment provider identified at checkout; AMT may receive transaction information needed for billing and records.
• Website and communication information that you submit or that may be generated through Website use, such as quote requests, forms, emails, IP address, device or browser information, pages viewed, and cookies or similar technologies.

If you provide information about another person, you represent that you have lawful authority to do so.

2. How We Use and Share Information

We use personal information to respond to inquiries; evaluate, arrange, and coordinate transportation; communicate with Patients, Clients, representatives, providers, and facilities; obtain medical, aviation, customs, immigration, and other clearances; process payments and insurance; maintain records; prevent fraud; protect safety; administer and improve our Website and services; enforce agreements; and comply with law.

We may disclose information, as reasonably necessary, to independent air, ground, and medical providers; receiving or sending facilities; insurers and payers; payment processors; service providers supporting AMT operations; professional advisers; government and regulatory authorities; and others involved in a Trip or required by law. International transportation may require information to be transferred to and processed in other countries.

AMT does not use or disclose PHI for a purpose requiring written authorization unless the individual authorizes it or law otherwise permits or requires the use or disclosure.

3. Website, Security, and Retention

The Website may use cookies and similar technologies for security, functionality, preferences, and analytics. Where required, consent or opt-out controls will be provided. Third-party websites and services are governed by their own privacy practices.

AMT uses reasonable administrative, technical, and physical safeguards appropriate to the information maintained. No system, transmission, or storage method is completely secure.

We retain information for as long as reasonably necessary for transportation, medical, legal, accounting, insurance, dispute-resolution, security, and compliance purposes, and as otherwise required by law.

The Website is not directed to children for independent use. Information about a minor may be submitted only by a parent, guardian, or other person with lawful authority.

4. General Privacy Rights

Depending on applicable law and your location, you may have rights to access, correct, delete, restrict, object to, or obtain a copy of personal information; opt out of qualifying sales, targeted advertising, or profiling; and appeal a denied request. AMT may verify identity and authority, request additional information, and deny or limit a request where permitted by law. AMT will not unlawfully discriminate against anyone for exercising a privacy right.

5. Notice of Privacy Practices for Protected Health Information

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN ACCESS IT. PLEASE REVIEW IT CAREFULLY.

How AMT May Use and Disclose PHI

When permitted or required by law, AMT may use or disclose PHI without separate written authorization for:

• Treatment and transport coordination, including evaluation, medical orders and clearance, communication with providers and facilities, and care before, during, or after transport.
• Payment, including benefit verification, claims, medical-necessity review, reimbursement, billing, and collection.
• Health care operations, including quality, compliance, training, credentialing, auditing, legal, administrative, and business-management activities.
• People involved in care or payment, when the individual agrees, does not object after an opportunity to do so, or disclosure is otherwise permitted in the individual's best interest.
• Public health, safety, and legal purposes, including abuse or neglect reporting, health oversight, workers' compensation, law enforcement, judicial or administrative proceedings, national security, organ donation, coroners, medical examiners, funeral directors, approved research, and preventing a serious threat to health or safety.
• Transportation and border requirements, including disclosures to aviation-security, customs, immigration, border, airport, and other authorities required for domestic or international transportation.

AMT may contact you about scheduled transportation and services related to your care or Trip. Uses or disclosures not described in this Notice or otherwise permitted by law require written authorization. Most uses of psychotherapy notes, marketing uses, and sales of PHI require authorization. You may revoke an authorization in writing, except to the extent AMT has already relied on it.

If AMT creates or maintains records protected by 42 CFR Part 2, those records receive additional protection. They generally may not be used or disclosed in civil, criminal, administrative, or legislative proceedings against you without specific written consent or a qualifying court order and subpoena, as required by law.

Your HIPAA Rights

When applicable, you may:

• Access or obtain a paper or electronic copy of PHI in a designated record set. AMT generally responds within 30 days, may charge a reasonable cost-based fee, and will explain any permitted denial and review rights in writing.
• Request correction of PHI you believe is inaccurate or incomplete. AMT may deny the request when permitted by law and will provide a written explanation, generally within 60 days.
• Request confidential communications by a reasonable alternative method or at another location.
• Request limits on certain uses or disclosures. AMT is not required to agree unless law requires it. When applicable, AMT will agree not to disclose information to a health plan for payment or operations when the information relates solely to a service paid in full out of pocket, unless disclosure is required by law.
• Request an accounting of certain disclosures made during the six years before the request.
• Obtain a paper copy of this Notice at any time.
• Authorize a personal representative to exercise your rights, subject to verification of authority.
• File a complaint with AMT or the U.S. Department of Health and Human Services Office for Civil Rights. AMT will not retaliate against you for filing a complaint.

AMT's Responsibilities

When HIPAA applies, AMT must protect the privacy and security of PHI, follow the Notice currently in effect, provide a copy on request, and notify affected individuals of a breach when required by law. AMT will limit uses and disclosures to the minimum necessary when that standard applies.

6. Changes and Contact

AMT may revise this Policy and apply the revised version to information it maintains. The current version and effective date will be posted on the Website and made available on request.
